Order Form

We are a small enterprise, and we can not absorb losses, so we do not take personal checks. We provide you three easy ordering options:

Option 1 -- Use Google Checkout here on the website, and your order will be charged to your Visa, MasterCard, Discovery, or American Express Card. No instructions are needed here, as they are provided during your checkout process. Google Checkout provides security and convenience, and you always have ample opportunity to review your order before it is placed. Option 2 -- Send your order by U.S. mail using the Order Form provided here. We accept Visa or MasterCard or Money Order by mail. To order by mail, click on the order form below, and it will open full size. Print out this order form, complete it, and mail it to Angler Innovations, PO Box 1620, Boring, OR 97009. Make sure you have included full credit card information or attach a money order.

Option 3 -- FAX your order to us using the Order Form provided here. We accept Visa or MasterCard by FAX. To order by FAX, click on the order form below, and it will open full size. Print out this order form, complete it, and FAX it to Angler Innovations at 503-658-6123.
